Chief Complaint:

Patient reported Headache, tiredness, blurred vision, increased urination increased thirst for the past 2 months

Vitals:

BP: 140/93

Pulse: 82

Temperature: 99.0

Labs Ordered:

Fasting glucose and HgB A1C

Lab results:

Results showed fasting glucose to be 101 mg/dl

Diagnosis:

Type 2 Diabetes, patient was referred to endocrinologist for followup and suggested weight loss and diet management.
