Chief Complaint:
Patient presented with chief complaint of polyuria, polydypsia, weight gain for the past 6 months. Patient has no past medical history and takes no medications. Family history was reported to have type-2 diabetes and hypothyroidism in mother. Patient has no other concerns.
Vitals:
BP: 120/80
Pulse: 85 bpm
Temperature: 98.2 F
Labs Ordered:
Labs were ordered for fasting blood sugar, HgbA1c, thyroid function test, vitamin B12 levels and vitamin D levels
Lab results:
Labs reported HgbA1c to be 5.8 and fasting blood sugar to be 119.76. Vitamin D levels were found to be 8.9 . TSH and B12 were in normal range.
Diagnosis:
Patient was advised to eat complex carbs and protein rich diet, lots of greens and vegetables. Patient wanted to avoid taking anti-glycemic medications and preferred to control her blood sugar through diet and weight management. Patient was advised to follow-up with PCP and repeat labs in 3 months, if no improvement medications need to be started if HgbA1c is found to be elevated
