Patient presented with chief complaint of feeling cold all the time, constipation, gaining weight and depressed for the past 6 months. Patient has no significant past medical history and takes multi-vitamins on a daily basis.

Pulse: 90 bpm
Temperature: 97.6 F
Labs Ordered:
Labs were ordered thyroid function test
Lab results:
Labs reported TSH to be 6.19 U/ml. T3 to be 0.1 and T4 to be 0.3
Diagnosis:
Patient diagnosed to have hypothyroidism. Patient is advised to see an endocrinologist and to start taking thyroid medications. Repeat the labs in 3 months. Exercise regularly and eat healthy food.
